There is a bit of information that we will need to collect from you to ensure the best possible health outcomes. To reduce our paper waste, operate as environmentally sustainable as possible, and increase security, we have digitized many of our processes. You can complete most New Patient papework via one of the following options:
Patient Portal
We offer all of our patients (new and existing) convenient access to their records through our online patient portal. There, you will be able to review/update your personal demographic information, update insurance information, fill out medical questionnaires (including new patient information), communicate with our staff, and submit requests for things like refills, referrals, and appointments. Simply go to portal.abamamd.com or click on “Patient Portal” at the top of this page.
New Patients:
- If you have already made an appointment with us, we should have your information on file. Look for an invitation to register for the Portal in your email or click “Enroll Now” on the Portal login page.
- If you have not yet made an appointment, you can pre-register online as a patient of our practice. On the Portal login page, click the “Pre Register” button and fill out all applicable fields. Once you register, our office will receive and review your information before your account is created. This can take a couple of business days. You will receive an email once your account has been created. You will then be able to complete the required questionnaires online.
Existing Patients: If we already have your information on file, including name, address, phone number, and email address, you can enroll on the portal by clicking on “Enroll Now” on the main login page.
All Patients:
Once you have a Portal account and are logged in, you can review/update your personal information by going to My Account > Personal Information.
To complete the New Patient medical questionnaires, go to Questionnaires and fill out Medical Questionnaire, Surgical and Allergies, and Immunizations.
Note: When filling out the Medical Questionnaire, be sure to choose the version that aligns with your sex at birth.
If you require any assistance creating a Portal account or need help logging in, please do not hesitate to contact our office.
Text Message
If you have provided us with your cell phone number, our system will send you automated reminders of upcoming appointments via text message. These reminders will include a link to check-in and complete most pre-appointment paperwork online, saving you time when you get to our office. You can check-in up to 3 days before your appointment.
If you have not done so already, please provide your mobile number to our staff and request Text/SMS notifications to use this feature.
Upon arrival, let the front desk know that you have already checked in from your phone.
Check-In Tablet
If you prefer not to use Portal or Text Messaging, you will be asked to sign all the necessary consent forms and answer all new patient questionnaires on our check-in tablets when you arrive. The questions are extensive, so we ask that you arrive 15-20 minutes prior to your appointment if you choose this option.
Upon arrival, let the front desk know that you are checking in and they will provide you with the tablet and offer any assistance you may need while using the device.
If you prefer to use this option, we strongly suggest reviewing the Preview document posted on the right of this page so that you are prepared to answer the questions.
